I remember reading that GRRM intentionally included incomplete maps with A Song of Ice and Fire to give the impression that the world had not thoroughly been explored, and to create the same sense of mystery about the rest of the world that the actual characters would have. I always think it's mad that only 700 years ago no-one in the east had any idea that the Americas existed, and life there was just, y'know, going on. Or that the Congo remained virtually unexplored until less than 150 years ago.
